MEDICAL POT CARDS TO BE ISSUED

STOCKTON - Medical marijuana users living in San Joaquin County can
apply for identification cards starting Monday.

The San Joaquin County Board of Supervisors in November approved the
implementation of the state-mandated program for the county to
provide identification cards for approved medical marijuana users.

The annual fee for the photo identification cards will be $141 per
year, and $71 for Medi-Cal recipients.

Applications can be obtained from the Vital Records Office at 1601 E.
Hazelton Ave. in Stockton or downloaded from the state Department of
Public Health Web site at www.dhs.ca.gov/mmp.

Patients must meet the following criteria:

Residency in San Joaquin County. Proof of identification. Written
documentation from a physician that the patient has been diagnosed
with a serious medical condition for which medical marijuana is
appropriate treatment.

Completed applications will be accepted by appointment only. Call
